Mayanei HaYeshua Hospital operates a center for assistance in realizing eligibility according to the National Health Law and the Patient Rights Law.
About the Center
Mayanei HaYeshua Hospital is a public medical center with agreements with all HMOs. The hospital provides medical services to members of all HMOs from all over the country. Treatment is given upon presentation of a commitment – Form 17 – and without additional payment.
If you requested a Form 17 for a medical service provided at “Mayanei HaYeshua” and your request was rejected, you can contact the helpline and we will be happy to help you exercise your rights!
The hotline is staffed by an experienced professional team, familiar with all the rights of patients and the various bureaucratic procedures, which are often known to applicants to undergo medical procedures in the various hospitals.

Important to know
A member of the HMO is entitled to choose a service provider for himself and for his registered child among the service providers of the HMO or on its behalf.
The National Health Insurance Law gives patients the right to choose the preferred place for themselves or their children to receive medical treatment. A member of the HMO is entitled to choose a service provider for himself and for his registered child among the service providers of the HMO or on its behalf.
This right is available to every citizen even when it is necessary to contact the emergency room by himself. If the patient has been hospitalized, the payment for the emergency room will be at the expense of the HMO. In case you have not been hospitalized, there may be cases of exemption from payment. The list of exemption cases appears in the Ministry of Health publications and in the HMO centers.
It is important to know that even if you have been given a referral for the ER at another hospital, you can contact the ER at the Mayanei HaYeshua Medical Center with this referral. In addition, by law, any patient referred to the hospital by MADA may choose the hospital to which they will be evacuated.
